Signing letters on behalf of someone else

if i am signing a letter on behalf of someone and that someone knows the person as Mr Smith but I know him as John do I change the salutation to Dear John or do I leave it as Mr Smith as although I know the person the sender does not

I would err on the side of caution and leave it as "Mr Smith."
