signing for my father

My dad had a stroke two years ago and still can't sign his name, so when we go to doctors appointments I sign for him because he can't and he tells me to. Nobody has had a problem with this except his new eye doctor. They told me I need league documents saying I'm p.o.a. to sign and said I can go to jail for this. Is that true even thou he told me I can sign for him?

There are places on the internet where you can get one (valid for your state), or get them at a local hospital.

If your father is mentally incapacitated, then you need to consult a lawyer because someone who is mentally unsound cannot execute such a document.
