Signature Lines and Encl/cc placement on the page

My letter is written by employee by direction of supervisor. The supervisor has been cc'd.

Is it correct to have this much information on first signature line? name, degrees, title

Does the name of the enclosed docment need to be included after the word 'Enclosure' and is the word correctly placed on the page between signature lines and cc?

Is the cc correctly placed on the page below the Enclosure line?
