what is SIGN-OFF FOR....

Someone who signs off on something shows acceptance (or at least awareness) of the information by signing his or her name near that information.

what is TO INITIAL AT ....

Sometimes people "sign off" on something by writing their initials rather than their full names. For instance, if my name is Pat Alan Hayes, my initials are PAH. So my writing "PAH" by various pieces of information would show that I had seen that information.

2. Rail Project ..... what does it mean?

I'd guess that it is a particular job involving a rail or rails.
