Sick leave letter

Hi all,

i would like to write a sick leave letter on behalf of my mum. she want to take 1 month unpaid leave due to healthy problem. below is my draft letter, can anyone help me to amend it?

1 Month Sick Leave Application

Dear HR Department,

I had been working in this company for past five years. I'm facing some healthy problems since this 1 year and suggested by doctor to take a rest. Hence, i would like to informed you that i will take 1 month unpaid leave to rest at home effective from 1 March 2010. i will continue back to work on 1 April 2010 if i'm recover.
