Should it be has or had ?

Can someone kindly point out whether it should be has or had over here:

1. He had been ill like this in the past
OR
2. He has been ill like this in the past.

It depends on the context.

If "like this" refers to now, i.e. he is ill now, then say "He has been ill like this in the past".

"He had been ill like this in the past" implies that "like this" is itself in the past, and "in the past" is even further back in the past. For example. "Three years ago he became ill. He had been ill like this in the past."
