Should I put my Letter of motivation in a separate envelope?

I have to send my application for university today, so I need a few quick tips...

Should I put my Motivation letter in a separate envelope, or just put it together with other documents I'm sending, as a separate paper without an envelope?

Also, should I write 'letter of motivation' somewhere as a title? And if yes, where should I write it? Before 'Dear Sirs', or before recipient's address, or?
