Should have already been there?

hi, i'm wondering which is correct one:

He should have already been there.
He should already have been there.

is there any difference between them?
if not, which is NORMAL?

and i have 2 more questions
-what happens if these sentences become questions??
-can you say "i should never have done that"? where should 'never' be?
