on earth emphasizes the uniqueness of her clothes, and also suggests that the speaker disapproves of her clothes, as if they were from another planet! I find it both disapproving and comical to add on earth in this sentence.

Typically, on earth is added to a question word:

Who on earth would want a gift like that?
Where on earth did you

I agree with CJ. In your sentence, you could also use "on the planet" for this sort of emphasis.

Though it's not absolutely necessary, I would personally feel more comfortable if you added the word "else":
Mr. TomShe wears the kind of clothes nobody else on earth wears.
