Setback/relapse/?

A - Is he still battling depression? He seems to be doing a lot better.

B - He is. Well, he had a small setback/relapse last week, but now...

A - A setback/relapse?

B - Yeah, he tried to kill himself.


Does one of "setback" or "relapse" seem natural here or will both words make you think of an alcoholic or a drug addict?
