Sentences to be included in a report ... help please!

I need to include these sentences in a report. Could you please, please help me by reviewing them:

1) The auditee was not aware of what should be done in the event of receiving a complaint from the client.

2) The auditee is to review the complaints procedure and familiarise with it.

3) The "complaint form" had not been completed as evidences were still being gathered.

4) The programme of the project X was included in the division’s main projects list but it was not updated. It was verified that the auditee sent the revised dates of the project X to the quality department (responsible for updating the main projects list), who will be updating the main programme accordingly.

What would be the meaning of the sentence # 3 if the comma is missing before ",as evidence"?

It would probably be interpreted the same way, Rishonly, because then we would have the past perfect standing alone with no other past event, and with the uncompleted form and the evidence still being gathered. Some logic is required, I think-- use commas wisely yourself, but never rely on them in others.
