Could you tell me which one is right among the following sentence? Thank you in advance.

1. How much did it cost you for a lawyer to set up a company by proxy of you?
2. How much did it cost you that a lawyer set up a company by proxy of you?
3. How much did it cost you when a lawyer set up a company by proxy of you?

1/3/4 are OK, but not 2.

The phrase 'by proxy of you' sounds wrong.
I'd say eg . . . set up a company for you by proxy. But I am unclear as to your meaning.

Can't you just delete the phrase, and say
eg How much did a lawyer charge you for setting up a company?

..

Then, if I ask a lawyer the legal fee directly, which one is right among the following sentences? I am not sure whether 'by proxy of me' is essential or not in case of this, but the reason why I add 'by proxy' to the question is just to make it clear that he/she acts on behalf of me for setting up a company. If it is not necessary, please tell me about it.

1 and 3 are fine. Not 2.

Don't say 'by proxy for me'. It is implicit that he would do it for you.
