I have a question.

''People with a strong immune system are most likely to recover quickly.''

People with = ?
Strong = Adjective
immune system = Subject
are = Linking verb
most likely to recover = adjective phrase

In my opinion, with a soft cotton cloth is a prepositional phrase/adjunct of instrument. A soft cotton cloth is a noun phrase/objetct to preposition.
