Sentence grammar

I work on wards as a pharmacy technician. Drug history taking is one of the main duties to establish what medicinies the patient was on or taking before coming to hospital- we call it meds on admission.

Today I had to ring the husband who was acting as a career for an elderly patient. The meds were stopped for some swallowing issues. The husband was not wether the GP or District nurses were concerned about her swallowing.

Does the following statement grammatically correct for my documentation.

The husband could not confirm who actually stopped or advised to stop the regular medicinies.
