SENTENCE CORRECTION - PLEASE HELP

JUNE has been under a Performance review which started on DATE and was concluded on DATE. This review was initiated due to JUNE failing to meet the quality KPI standard of 50% as well as her failure to comply with procedures pertaining to her markets requirements. During this period JUNE received 3 daily call evaluations with Feedback. Once weekly we met with JUNE to review her statistics as well as highlight the areas of concern. With each feedback session, JUNE agreed to use the feedback given to better her scores. This, however, was not the case and a pass rate was not achieved.
