Sentence check

i didn't know they were married for four years.
i didn't know they are married for four years.
I didn't know they have been married for four years.
I didn't know they had been married for four years. - Does this mean they are not married anymore?

Please tell me what are the differences between them.

Also, please tell me if these are both right:

I didn't know that you had an appointment with Dr. Smith tomorrow.
I didn't know that you have an appointment with Dr. Smith tomorrow.
