Sentence Check

Please let me know which sentence is correct. Also should I mention "describe the transaction" only once in the sentence as done in the second sentence as compared with the first one.

• In the Short Comment field, enter the comment to describe the transaction and/or in the Long Comment field, enter the additional comment to describe the transaction.

• In the Short Comment field, enter the comment and/or in the Long Comment field, enter the additional comment to describe the transaction.

I would break this into two sentences for clarity sake (and start with the action rather than the place of that action). Also, do you really mean "and/or"? That is, can a person put a comment in either field (short comment or long comment) or in both fields?
