Self-conscious

I have doubts about the expression "self-conscious"

If I write this: "to promote a self-conscious social integration of the person"

Does it mean that the person is fully conscious of what he is doing??

It "sounds" to me more like the person is shy about that integration, or insecure, or uncomfortable

Am I right?
