Is it possible for this sentence below to mean that he is self-centered?

"He thinks about himself only" or "... only about ..." would do fine. You / we are here playing with the positions of the word "only".

"only thinks" means "does nothing else"
"only he" would mean "nobody else"

"He's an egoist" simply says "He is self-centered".
