Our Intern Dr. Mohammad has expressed a deep interest in doing his Elective Rotation in ENT at your department under the supervision of Dr. Hashim during the month of April 2011.
In this, we would like to request from you to accept him on the above-mentioned period. In addition, our secretary will send soft copy of evaluation form to be filled up at the end of rotation and sent back directly to preserve the data authenticity.
We will appreciate more if you can provide us an Official Letter of Acceptance even its electronic mail at your convenience. Hoping for you kind approval and thanking you in advance for your continuous support.
