I don't know what 'tickbox plus declarations' means, so i don't know if it makes sense to scan them.

Frankly, the original UK document is badly written. I noticed a spelling error on a title. Nonetheless, I think "scanned signatures and tickbox plus declarations" means they will accept your scanned signature (you sign a form and scanned it with your signature) but you must also put a checkmark on some tickbox (perhaps a tickbox that says "All information I disclose in this form are accurate; othe
