Should it be:

Hair in scalp. Or

Hairs in my scalp are falling out. Or

Hairs on my scalp are falling out.

I suppose both sentences to be incorrect

As I understood you are trying to use scalp as = the skin covering the head, excluding the face...Ex:

Massage the shampoo into the scalp
But as you write about hair... you shouldn't use word scalp in your sentence at all

A scalp is a surface so in my opinion you'll have to use on.
