Saying dates in English

Hello,Are all  of these examples  correct?   And which of these examples are acceptable in American English? Her birhtday is  October 15th. Her birthday is  on october 15,  2011.  Today it's October 15, 2011.Today it's  25 November.It happened on 25 September 2011.It happened  on September 25rd, 2011. I noticed that the 'on' 'the' 'of'  can be omitted sometimes, but I don't know when.Thanks

It happened on 25 September 2011.

It happened on September 25th, 2011.

I noticed that the 'on' 'the' 'of' can be omitted sometimes, but I don't know when.-- 'On' can usually be omitted. 'Of' is used only in the structure "the 25th of Month'.

Dates are difficult, and many times - particularly in American English - you'll see things like 'the meeting is scheduled for 27 November'. Business English can be very challenging with 2011 10 21 (today's date) making inroads to daily office use. I think that if you stick to the formulas above, you won't have any problems.

In spoken AmE, you need to use "the" and "of" if the number is first:

It's the 25th of September.

If the number is last, you don't need "the" or "of":

It's September 25th.
