I'm not MH, but I think I can answer your question: only (3) Dear Mr. Lastname is correct and formal , although unsolicited junk mail-- and some more reputable mass mailings-- have recently taken to the unfortunate habit of using 'Dear Firstname', which I find not merely too casual, but downright rude.
