Salary Increment

Hi, Please find below the letter whihc i need to send it to my manager for salary increment, i have informally discussed with hbut he asked me to put in writing, If you can check the grammar and english,

Dear <Manager>,
Hope you are well.

As per our discussion last week with in regards to the salary review, I feel that this is the appropriate time because we agreed to review my salary after the first three months of my probationary period but this has been prolonged to five months.
I believe that an increase in my salary is now warranted, because I have successfully taken responsibility for managing the accounts department of the company and have resolved many outstanding issues since I have been taken on board. Especially, great role has been played in the most outstanding issue of utilities which has tested my patience but finally after 5 months of struggle we are in a position where we have full list of utilities which company was liable for.
My knowledge and skills in Finance and Accounts has allowed producing monthly Cash Flow Statement and Management Accounts which gives an overview the company's profitability, aged debtors and creditors report. I assume and handle management responsibilities, traditionally highly paid work.

I believe that going forward my responsibilities will be producing monthly Management Accounts, comparing our financial performance with that of forecasted. Also, Monthly Landlords Statements would be produced in order to make payments to the landlords, raising monthly invoices, Maintaining Accounts Receivable and Payables, Monitoring Cash Flow and other ad-hoc duties.

I believe that I have demonstrated a commitment to the company and on the basis of my above accomplishments and responsibilities going forward my salary expectation is £X per day, which I assume is reasonable for the responsibilities I will hold.

I have been led to believe that you are satisfied with my work, so I am confident that you will want to sincerely consider my request.

A per diem salary is very odd for management. Shouldn't you be asking for £X per month ?
