Salad in "a bowl" vs "in bowl"

1) lets say you are ordering a salad at Mcdonald's.

And the server asks you to choose between a wrap and a bowl.

Would you say, "in bowl please!" or "in a bowl please!"

2) Shall vs. should

why do people use shall for shall we dance instead of should?

Either say "bowl, please" or "in a bowl, please" but not "in bowl, please"
