~said looked like~

My little girl piped up, "Mommy, is this the lady you said looked like a horse?" I wanted to die.

I don't understand the underlined part..

Is it not like: the lady you said who looked like... ?

I don't understand how a past verb "looked" can come after say...

The sentence is in the past tense. :-
..... you said that she looked like a horse.

Mother (at some time in the past) "she looks like a horse"

Here is the lady who looks like a horse.
Here is the lady you say looks like a horse.
Here is the lady you said looked like a horse. (or) you said looks like a horse. Either is correct.
