Although my grandfather is old, he is not rusty. The speaker probably means feeble or deteriorated in some way.

However, when I hear the word 'rusty', it is always in connection with some specified skill.

Odd, but the first thing that sprang to my mind was: Although my grandfather is old, he is limber.

All of the other explanations are perfectly correct and probably preferable.

I just felt that if the gentleman was not rusty, he must be lithe, supple or flexible.
