Reviewing

In this role you will have a key part to play in:

  • Support the role of the Clinical Pharmacist at GP federation level on the delivery of the new Clinical Pharmacy in General Practice model in Wakefield (CPGP)
  • Carry out medicines switches in GP practices in line with practice and CCG agreed specific protocols – reviewing patients’ records; amending records and informing patients

I saw the above in NHS job description.

What is the grammatical form and function of the part-reviewing....informing patients?

It looks like three verb phrases separated by commas but I can't figure out it's relation at sentence level.
