Requirements List
I'm working now, as person in charge who answer many questions from customer of my company. Frequently I have to inform some requirement that customer must follow ... How to write this in a good sentences ?
I attach below .. one of them, may I write better ?

Thank for your email to us,
We have processed the disconnection request aforementioned to our related department. The schedule is on Monday October 25th according to your dealing with our agent Mrs.Iim.
Due to your request to refund the converter deposit, we have requirements as below :
1. Work Order for collecting converter ( you will sign it when our technician disconnect your cable TV on the field)
2. Request Letter for refund ( include the number of your bank account transfer, the branch of your bank, in here it must mention your name not any one else )
We will process this refund approximately in 1 month, start when the work order of collecting converter closed by system
Thank for your deeply attention, and we need your response to complete the requirements above (you can choose send them by email, fax, or letter of course)

I think it just means that the refund will only be made to the claimant's account, not to another person's (third party) account.
