Requesting letter

help us to write a requesting letter to the assistant dean of arts & sciences to give us a preliminary exam because one of our professor in our minor subject didn't gave us a preliminary exam because we didn't show a permit and these is not fair for us because he didn't inform us ealier before the said exam. in our school, the only time we need to show our permit is in midterm & final examination.
