Request for Re-schedule in Leave Date

Dear Sir,

My wife will go through a surgical process for her delivery, which is schedule to be on 11 Jan 2018, for that I requested 10 days (10/02/2018 to 20/02/2018) leave from my boss and it is granted.

Hence my wife having severe post pain and she is hospitalized and The Doctor re-scheduled the date of surgery, which will be done on 05 jan 2018.

Therefore I want your help to write a good draft for me, so I can submit to my boss to request for reschedule my leave date instead of 10 Feb 2018 to 05th Feb 2018

Pls my leave has been rescheduled and it's a negotiation between myself and my boss. He said I should take a half leave so he would add the other half to next years. I want to write a letter of evidence so he cannot changer his words next yr. Pls kindly help me out. Rescheduled date not out yet, original date is 8th/10/2018 to 15th/10/2018.
