Req for a 10 days leave for wife delivery

Dear Sir,

I want to request for 10 days leave for my wife delivery, kindly suggest a good draft for me.

My wife EDD is on 22nd Feb but due to some complications her gynecologist recommend a surgical delivery on 10th feb.

So, I have to explain this all to my boss, otherwise he will not grant me a leave.
