reported speech?

I sometimes hear sentences in the movies which seem like reported speech, but they do not follow the rule, such as " tell me what's wrong?" or " tell us what does it mean?". Why don't they say " tell us what it means" for example? It's a bit confusing to me, when and where to report, though I know the grammatical rule.
