Report for appeal application

Please help me improve the following report for an appeal application. Thank you.

This office has received a verbal appeal application from customer, Mr. Galloway, through our customer services center on 5 July 2016. Mr. Galloway was subsequently contacted by phone on 7 July 2016 and was asked to submit a written appeal application.

Upon receiving his letter on 9 July 2016, we understood that Mr. Galloway had no clear memory whether or not he signed in when using his reserved tennis court from 1500-1700 on 5 July 2016, but he insisted that he did borrow the racquets from our equipment counter and signed on the undertaking for use of our equipment in the meantime. Details of Mr. Galloway's booking are as follows:

Since this is the first time Mr. Galloway having found no-show within three months time, he will not be suspended for reserving our facilities unless another no show scenario arises. After reviewing the telephone conversation and the letter from Mr. Galloway, in which the signature indicated on the letter does equivalent to that on the undertaking for the use of our equipment. Therefore, we trust that there are sufficient evidence to certify Mr. Galloway's presence at the tennis court on 5 July 2016 and his appeal for lifting the no show record is considered to be justify.
