Reply to explanation letter

i have been served an explanation letter wherein i have been charged with opening bank accounts with false/fabricated documents from my bank.the charge is not true and neither has my bank been able to retrieve the documents to prove its charge. they say that some personal loans have gone bad due to my lapses in opening the accounts. my controller has not furnished any document with the explanation letter to prove charge. i have been asked to explain in 5 days. i intend to reply with a polite but firm letter. please help.
