replace

Hello.
Yesterday in my class my teacher was talking about a basketball game. She said both of these sentences are correct:
Kukoc replaced Jordan. OR Jordan replaced by Kukoc.

Do they mean the same?
And I think the second sentence means Kukoc replaced Jordan with someone else(?)

Thank you Mister Micawber.
Jordan wasn't playing in the game and Kukoc was in the game.So if the coach replaces a player, I can say:
Kukoc replaced Jordan.
Jordan was replaced by Kukoc.

And can I use 'replace with'?
