Reminder for promised award

My name is Wahid, my position is data entry, I worked hard, so hence the head office appreciated me and they promised for a good award for me, but I didn't get yet, so I want to send a reminder letter as below my written letter, Can anyone, please help me and check my mistakes.

Hello sir,
My name is Wahid, employee of XYZ Company as data entry position in XYZ, I would like to inform you that promised award to me via email (2015-6-3), because I got first grade as hard worker over all provinces so I am requesting from your respect department about mention promised awards if it's possible so it will be good heartening for me.
