Refferance letter formate

Hi everyone,

I was working in an indian company for a UK process, and now need a refference letter from UK company which declare that i have worked for this time period with them and my work profile as well some good points about my work.

I was working for SEM (Search engine marketing) process...

Can anyone suggest me a good and effective format for the same.
