Referential concordance

Hi. In Ephesians 6:17 of the Bible, in the New International Version, it is stated:

Take the helmet of salvation and the sword of the Spirit, which is the word of God.

1) Which is the referent of the pronoun "which"? Is it both the helmet of salvation and the sword of the Spirit, in which case I think the pronoun "which" should be followed by "are"?

Also, in Matthew 7:14 of the Bible, in the New International Version, it is stated:

But small is the gate and narrow the road that leads to life, and only a few find it.

2) What is the referent of the pronoun "that"? Is it both the gate and the road? If yes, should it be "that lead"? instead of the singular "that is"?
