Refer to?

Hi! everyone.Could you tell me,what does "refer to" mean in the following scenario?
I see people on web(particularly on English forum)after giving some kind of context asks these kind of questions-:What does "it"refers to?
Or what does"they" refer to?
Now I want to know what "refer to"mean in the above questions(asked by peoples)?
Could "be assigned to" or be designated to"be the substitute words for "refer to" in the above questions?
And Please tell me,in which context "refer to" can mean "be assigned to" or be designated to?
The reason I am emphasizing on 'assign to' and "designate to' to be the synonyms of "refer to" is I saw these two words in the synonyms of
"Refer to".
I will be very thankful,if you explain this to me and allay my confusion about that use of "refer to"
