Ref. to hard copy of a form, will it be "on" or "in"?

When i am referring to filling up something in the hard copy of a form, would i say "In" the XYZ form, complete.... . Or would I say, "On" the XYZ form, complete... . Or has this got anything to do with British/American English? Please do let me know.
