Really

I have a question about this kind of rhetoric:

A: I pooped my pants!

B: Really!?!?

A: Yes, VERY really!

It's bad grammar, but can it be used?

Is it because:

Very is an adjective/adverb and Really is also an adverb?

'very' is an adverb of degree. It says that the adjective or adverb applies to a great degree. 'very skillfully' ~ in a manner that is skillful to a great degree.

'really' is not gradable. Once a thing is real (~ true), it is not fake or false. It can't be a little real or greatly real (true).
