I am currently drafting an email for a former professor asking to use her as a reference for an internship. Would the following be acceptable and where can I make improvements? (She is a communications/english professor)

Dear Professor Smith:

Will he /she remember you? Do you need to mention the class name and the dates that she taught you?

I hope this email finds your new semester advancing as well as mine is. I am writing in regard to an internship I am applying for. The internship application requires that you I have the email address of a reference who can discuss your my academic success if the application committee needs more clarification. While I do not know that you would actually be contacted if you consent, I was wondering if I could list you as a reference due to my success in your class. Thank you for your time in advance.
