Rate vs ratio

Is it correct to refer to a certain percentage as "ratio"?

e.g. The failure ratio was 40%.

If yes, could we use either "rate" or "ratio" in this case?
Do they mean the same thing?

I thought ratio was something like 5:3.

Thank you!
