It means "is that all?", "is that everything?", "are we finished?". It has a slightly humorously rude air.

Here's one possible context of many. Imagine you're at a good friend's house. She offers to make you something to eat. She puts the plate in front of you, and you see a very small sandwich. You stare in humorous disbelief at the plate and say "is that it, then?"
