"How came"!?

I found several sentences in my grammar book which I think is strange. They are as follows:

How came you to know him? (My book says it equals to "How did you come to know him?" Is that really true?)

A: I was late for work this morning.

B: How came? (Shouldn't it be "How come"?)

A: I missed my bus.

Please help me with this question, thank you!

Your first example is real English but an archaic form of the past interrogative. We would not say 'How came you?" now. Your second example is a misprint-- it should be as you said: "How come?"
