"How are you doing?"

How to respond to this question? Is it appropriate just to respond by saying "Great" " Fine" or should I say something about where I am and what I am doing?
And is there any difference between "How are you doing?" and "How are you?"

It's not asking where you are or what you are doing.

How are you doing? is informal as a greeting. How are you? is slightly more formal.
