I'm having some difficulty with the proper grammar of this sentence for my medical transcription work. Can you help me out?

He reports that his condition now feels even, “worse”.

or

He reports that his condition now feels even, "worse."



I can't figure out the rules on quotations when it comes to this sort of work.
