Questions on the usage of ariticles and punctuations for a list of items

Hi there,

Currently, I am writing a cover letter for my graduate school application package. In the cover letter, I want to list all the items enclosed, like:

...
Enclosed please find:
-- A resume.
-- A reference letter from professor Tough.
-- A completed application form.
....

So my questions are:
1. Should I use "a/an" for each item? Actually I do see people omitting articles, but is it the formal way?
2. What is the formal usage for the punctuations at the end of each item?

Personally, I think that I would write it as a sentence within the main text:
